Skilled Nursing
 
On average, if after 3-7 days on the acute floor, you are not yet ready to return home, you may be transferred to Covenant Medical Center or Sartori Memorial Hospital's Skilled Nursing Unit for additional care.

The goal of the Skilled Nursing Unit is to assist you in recovering and maintaining your strength, mobility, comfort, and independence- all factors that help you reach your optimal level of health.

Patient Responsibility and Independence
An important objective of the Skilled Nursing Unit is to enable you to become as independent as possible, so that you can make a smooth transition back home or to the next level of care. Therefore, there is a different philosophy of care between the Skilled Nursing Unit and the acute care areas of the hospital. On the Skilled Nursing Unit, you will be expected to assume increasing amounts of responsibility for yourself.

Personal Care and Comfort
The Skilled Nursing Unit will encourage you to be up and dressed as you would be at home, so you will need several changes of clothing. You will need to make arrangements for your personal laundry to be done outside the hospital.

Meals
Meals are served in the Skilled Nursing Unit at approximately 7:30am, noon, and 5:30pm. If you are able to be out of bed, you may wish to eat in the dining/activity room. You may also schedule gatherings with family or friends in this area.

Physician Rounds
Each resident will be seen by their attending physician at least once every 7 days.

Activities and Passes
Scheduled group and individual activities are planned as part of your rehabilitation and for your enjoyment by the recreational therapists. If you are medically able, you may go outside or spend periods of time away from the unit, with your doctor’s permission.

Visiting Hours
Visiting hours are the same as for the general hospital (11:00am-8:00pm). However, flexibility is allowed for family and friends involved in learning about your care.

Planning Your Discharge
Within a few days of your admission, the skilled nursing staff will visit with you and your relatives, or interested persons, about planning the steps necessary to expedite your discharge. They want you to be actively involved in this care planning process and encourage you to offer suggestions and inform them of your special needs.
